Objectivos: Avaliar o efeito de um projecto de base comunitária e familiar - Projecto Obesidade Zero (POZ) - no estado nutricional das crianças bem como no número de horas de sono diárias, na prática desportiva e no visionamento televisivo/utilização de computador em crianças de idade escolar. Metodologia: O POZ foi desenvolvido durante o ano de 2009 em cinco municípios de Portugal (Melgaço, Mealhada, Cascais, Beja e Silves) com articulação entre as cinco câmaras municipais e os respectivos centros de saúde. O estado nutricional das crianças foi classificado com base nas curvas de percentis de Índice de Massa Corporal (IMC) para o sexo e a idade do CDC (2000) e os dados sobre as variáveis em estudo foram retirados do questionário sobre hábitos alimentares, comportamentos e conhecimentos dirigido aos pais das crianças. Foram realizadas estatísticas descritivas e analíticas. Low levels of physical activity, short sleep duration and the increase of sedentary behaviors are associated with childhood obesity. Objectives: Assess the effect of a community and family based project - Project Obesity Zero (POZ) - in nutritional status of children and sleep duration, physical activity and television viewing/use of computer in school-age children. Methodology: POZ was developed in 2009 in 5 municipalities of Portugal (Melgaço, Mealhada, Cascais, Beja and Silves) articulated with Healthcare Centres and local governments. Nutritional status of children were classified according to the age and gender specific and Body Mass Index (BMI) 2000 CDC Growth Reference and the information about sleep duration, physical activity and television viewing/use of computer was collected from the questionnaire about food habits, food behaviors and nutrition knowledge directed to parents. After intervention, children that participated two or more days per week in a sport club (88,2%), children that spent less than two hours per day during the week watching television or using computer (70,3%) and children who slept ten or more hours per day during the weekand (66,3%) decreased their BMI percentile/age. <br/>Conclusions: This project seems to confirm the efectiveness of community and family based projects in the prevention of childhood obesity, enphatizing the importance of increase sleep duration and exercise and decrease sedentary behaviors like watching television and using the computer in order to improve nutritional status of children.</p>     <p><b >keywords</b>: Physical activity, Television, Computer, Sleep duration, Project Obesity Zero</p>     <p>&nbsp;</p><hr>    <p>&nbsp;</p>     <p><b >Introdu&#231;&#227;o</b> <br/>&#201; largamente conhecido que a obesidade infantil &#233; considerada pela Organiza&#231;&#227;o Mundial de Sa&#250;de (OMS) como um dos principais problemas de sa&#250;de p&#250;blica do s&#233;culo XXI (1). Mundialmente, segundo a <i >International Obesity Task Force</i>, uma em cada 10 crian&#231;as com idades compreendidas entre os 5 e os 17 anos tem excesso de peso, sendo 30-45 milh&#245;es obesas (2). Em Portugal, de acordo com os resultados da primeira fase do estudo COSI, a preval&#234;ncia de excesso de peso em crian&#231;as dos 6 aos 8 anos de idade, com base nos crit&#233;rios do <i >Centers for Disease Control and Prevention</i> (CDC), foi de 32,2%, sendo 14,6% obesas (3). Uma crian&#231;a com excesso de peso tem um risco acrescido de se tornar num adulto obeso com co-morbilidades associadas que diminuem a sua qualidade de vida (4). Neste sentido, a abordagem desta doen&#231;a representa uma prioridade. Uma abordagem que envolve toda a comunidade (5) e contribui para a sensibiliza&#231;&#227;o dos indiv&#237;duos e consequente educa&#231;&#227;o para um estilo de vida mais saud&#225;vel, tem potencial para influenciar os determinantes da sa&#250;de ao n&#237;vel social e econ&#243;mico de um modo flex&#237;vel, sustent&#225;vel e equitativo (6-8), tal como foi demonstrado nos projectos <i >Be Active Eat Well </i>(7, 8), <i >Shape Up Somerville</i>: <i >Eat Smart, Play Hard</i> (9) e <i >APPLE</i> (10). A abordagem da obesidade infantil deve envolver toda a fam&#237;lia, incluir educa&#231;&#227;o alimentar, mudan&#231;as comportamentais e promo&#231;&#227;o da actividade f&#237;sica (5, 11). Nas crian&#231;as, o exerc&#237;cio f&#237;sico, medido atrav&#233;s de v&#225;rios par&#226;metros como a frequ&#234;ncia de actividades desportivas, &#233; particularmente importante, uma vez que melhora a aptid&#227;o f&#237;sica e a sa&#250;de &#243;ssea bem como diminui a gordura corporal e os sintomas de depress&#227;o (12, 13). Segundo as recomenda&#231;&#245;es da OMS, as crian&#231;as dos 5 aos 17 anos devem praticar pelo menos 60 minutos de actividade f&#237;sica di&#225;ria e devem restringir os comportamentos sedent&#225;rios a menos de duas horas por dia (12). Para al&#233;m disto, a OMS (12) e o CDC (13) sugerem o envolvimento das crian&#231;as dos 5 aos 10 anos em actividades fortalecedoras dos m&#250;sculos 2 a 3 dias por semana. Por outro lado, o aumento dos comportamentos sedent&#225;rios, como o visionamento televisivo e a utiliza&#231;&#227;o de computador bem como a diminui&#231;&#227;o da pr&#225;tica de actividade f&#237;sica contribui para altera&#231;&#245;es no padr&#227;o do sono das crian&#231;as (14). Segundo o CDC, as crian&#231;as entre os 5 e os 10 anos de idade devem dormir entre 10 a 11 horas por dia (15). As crian&#231;as que dormem menos de 9 horas por dia t&#234;m um maior risco de desenvolver obesidade comparativamente com as crian&#231;as que dormem mais de 11 horas por dia (16-18).</p>      <p><b >Objectivos</b> <br/>O presente estudo tem como principal objectivo avaliar o efeito de um programa de base comunit&#225;ria e familiar &#8211; o Projecto Obesidade Zero (POZ) - no estado nutricional bem como no n&#250;mero de horas de sono di&#225;rias, na pr&#225;tica de actividades desportivas e visionamento televisivo em crian&#231;as de idade escolar.</p>     ]]></body>
<body><![CDATA[<p><b     >Metodologia</b>     <br/>O     presente estudo deriva da an&#225;lise de vari&#225;veis integradas no estudo POZ. O POZ     foi desenvolvido em cinco munic&#237;pios de Portugal (Melga&#231;o, Mealhada, Cascais,     Beja e Silves) com articula&#231;&#227;o entre as cinco c&#226;maras municipais e os     respectivos centros de sa&#250;de, durante o ano de 2009. Este projecto dirigiu-se a     crian&#231;as com excesso de peso e obesidade e compreendeu um programa integrado de     obesidade infantil ao n&#237;vel familiar.     <br/><u>Amostra</u>:     ]]></body>
<body><![CDATA[A selec&#231;&#227;o das crian&#231;as com excesso de peso foi realizada por cada munic&#237;pio e     foi, na sua maioria (97%), de base escolar (escolas do 1.&#186; Ciclo do Ensino     B&#225;sico), sendo as restantes crian&#231;as (3%) encaminhadas pelos respectivos     Centros de Sa&#250;de, para o programa POZ. Das 2226 crian&#231;as avaliadas em peso e     estatura, foram seleccionadas 482 crian&#231;as (percentil de IMC<u>&gt;</u>85) contudo,     189 n&#227;o aceitaram participar no projecto (39,2%).      <br/><u>Desenvolvimento     do Projecto</u>: Em cada munic&#237;pio foi indicado um     Respons&#225;vel Municipal que em conjunto com a Coordena&#231;&#227;o Cient&#237;fica do projecto     seleccionou e convidou cinco nutricionistas para integrar a equipa do POZ (um     ]]></body>
<body><![CDATA[nutricionista por cada munic&#237;pio). De forma a uniformizar a interven&#231;&#227;o dos     nutricionistas, a Coordena&#231;&#227;o Cient&#237;fica do projecto realizou duas sess&#245;es de     treino em Avalia&#231;&#227;o Nutricional Infantil e Alimenta&#231;&#227;o Saud&#225;vel na Inf&#226;ncia     dirigidas aos cinco nutricionistas. O POZ compreendeu as seguintes fases de     desenvolvimento: 1) Aconselhamento alimentar individual ao n&#237;vel das Consultas     de Obesidade Infantil (COI) &#8211; cada crian&#231;a foi convidada a participar em quatro     consultas de obesidade infantil, ministradas por nutricionistas. O     aconselhamento alimentar foi feito com base na Dieta do Sem&#225;foro. Paralelamente     foi promovida a pr&#225;tica de actividade f&#237;sica de pelo menos 60 minutos por dia,     bem como a redu&#231;&#227;o dos comportamentos sedent&#225;rios a um m&#225;ximo de 2 horas por     ]]></body>
<body><![CDATA[dia. A mudan&#231;a comportamental como parte integrante da abordagem da obesidade     infantil foi preconizada pelos nutricionistas ao longo de toda a interven&#231;&#227;o     (19); 2) Workshops de Cozinha Saud&#225;vel &#8211; cada crian&#231;a foi convidada a     participar num workshop de cozinha saud&#225;vel, acompanhada por um ou dois membros     da sua fam&#237;lia. Os principais objectivos foram promover e aumentar as     compet&#234;ncias em mat&#233;ria de culin&#225;ria saud&#225;vel, em fam&#237;lia. Os workshops foram     conduzidos por um Chef de Cozinha, previamente treinado pela coordena&#231;&#227;o     cient&#237;fica do projecto. Destes workshops resultou o Livro de Receitas POZ que     foi oferecido posteriormente a cada fam&#237;lia; 3) Aconselhamento alimentar em     grupo &#8211; cada crian&#231;a foi convidada a participar em duas sess&#245;es de     ]]></body>
<body><![CDATA[aconselhamento alimentar em grupo implementadas em ambiente escolar. O foco da     interven&#231;&#227;o foi a promo&#231;&#227;o de h&#225;bitos alimentares mais saud&#225;veis, nomeadamente     no que concerne &#224; diminui&#231;&#227;o do consumo de refrigerantes e ao aumento do     consumo de hortofrut&#237;colas e de um estilo de vida mais activo; e 4) Sess&#245;es de     aconselhamento alimentar em grupo dirigidas &#224;s fam&#237;lias &#8211; cada fam&#237;lia foi     convidada a participar numa sess&#227;o de grupo, na qual os nutricionistas     procuraram sensibilizar as fam&#237;lias para a import&#226;ncia da adop&#231;&#227;o de uma     alimenta&#231;&#227;o saud&#225;vel e de um estilo de vida activo na preven&#231;&#227;o da obesidade infantil     bem como procuraram encorajar e dar suporte &#224;s fam&#237;lias na adop&#231;&#227;o de uma     alimenta&#231;&#227;o mais variada e equilibrada em casa.      ]]></body>
<body><![CDATA[<br/><u>Estado     Nutricional</u>: O peso e a estatura foram as medidas     seleccionadas para avaliar o estado nutricional das crian&#231;as. Os instrumentos     antropom&#233;tricos (balan&#231;a digital SECA<sup>&#174;</sup> 840 com uma precis&#227;o de 0,1kg     e estadi&#243;metro SECA<sup>&#174;</sup> 214 com uma precis&#227;o de 0,1cm) foram     previamente calibrados e entregues a cada munic&#237;pio. Atrav&#233;s das medidas de     peso e estatura foi calculado o &#205;ndice de Massa Corporal (IMC), sendo a     estatura utilizada a m&#233;dia das duas estaturas medidas em cada crian&#231;a. Para a     classifica&#231;&#227;o do estado nutricional foram utilizadas as curvas de percentis (p)     de IMC para a idade do CDC desenvolvidas para crian&#231;as e adolescentes dos 2 aos     ]]></body>
<body><![CDATA[20 anos de idade, as mesmas adoptadas pela Direc&#231;&#227;o-Geral da Sa&#250;de e que     constam do Boletim de Sa&#250;de Infantil e Juvenil (20). As medi&#231;&#245;es     antropom&#233;tricas foram efectuadas nas quatro COI de acordo com as normas     estabelecidas no Manual de Consultas de Obesidade Infantil, desenvolvido para o     efeito, que seguiu as orienta&#231;&#245;es da publica&#231;&#227;o de Rito e col (21), as mesmas     preconizadas pela OMS.      <br/>Horas     de sono di&#225;rias, actividade desportiva e visionamento televisivo/utiliza&#231;&#227;o de     computador: Para obter a informa&#231;&#227;o sobre estas vari&#225;veis, aplicou-se o     question&#225;rio sobre h&#225;bitos alimentares, comportamentos e conhecimentos dirigido     ]]></body>
<body><![CDATA[aos pais das crian&#231;as. O question&#225;rio foi preenchido pelo nutricionista de cada     munic&#237;pio na primeira e na quarta COI. Para avaliar o efeito do programa sobre     as vari&#225;veis seleccionadas, foi verificado se do primeiro momento (1.&#170; COI)     para o &#250;ltimo momento (4.&#170; COI) o p calculado de IMC/idade de cada crian&#231;a     aumentou ou diminui. Para avaliar a pr&#225;tica de actividade desportiva foram estudadas     duas vari&#225;veis: 1) participa&#231;&#227;o das crian&#231;as num clube desportivo e 2) n&#250;mero     de vezes por semana que as crian&#231;as frequentavam esse clube desportivo. No que     concerne ao n&#250;mero de horas de sono di&#225;rias foi perguntado aos pais, em m&#233;dia,     a que hora &#233; que a sua crian&#231;a se levantava e deitava durante os dias da semana     e durante os fins-de-semana.     ]]></body>
<body><![CDATA[<br/><u>An&#225;lise     Estat&#237;stica</u>: Utilizou-se o programa <i     >Statistical Package for Social Sciences</i><sup>&#174;</sup>     (SPSS) vers&#227;o 18.0 para <i >Microsoft Windows</i><sup>&#174;</sup>     para a constru&#231;&#227;o da base de dados e posterior an&#225;lise estat&#237;stica. Foram     realizadas estat&#237;sticas descritivas como m&#233;dias e desvio-padr&#227;o (DP) para     vari&#225;veis quantitativas. Para vari&#225;veis qualitativas, utilizou-se     essencialmente contagens e propor&#231;&#245;es. Para verificar a exist&#234;ncia de     associa&#231;&#245;es entre as vari&#225;veis em estudo e a altera&#231;&#227;o de p entre a 1.&#170; COI e     4.&#170; COI, utilizaram-se os testes Exacto de <i >Fisher</i>     ]]></body>
<body><![CDATA[e Qui-Quadrado. O valor m&#233;dio entre duas amostras emparelhadas foi estudado     atrav&#233;s do teste de <i >Wilcoxon</i>.     Considerou-se existirem diferen&#231;as estatisticamente significativas quando     p<0,05.</p>     <p><b >Resultados</b> <br/>Das 293 crian&#231;as que aceitaram participar no POZ, 219 conclu&#237;ram o projecto. As principais raz&#245;es para o drop-out foram aus&#234;ncia do termo de consentimento informado ou recusa dos pais em participar nas sess&#245;es previamente programadas. 152 (52%) eram do sexo feminino, sendo a m&#233;dia de idades de 8,6 anos (&#177;1,4 DP). Relativamente aos resultados gerais do POZ, o p m&#233;dio calculado de IMC/idade no 1.&#186; momento foi de 93,6 (&#177; 5,9 DP) e no &#250;ltimo momento de 91,3 (&#177; 9,2 DP) (<a href="#t1">Tabela 1</a>), significando que 80,4% das crian&#231;as diminu&#237;ram o seu p de IMC/idade (p<0,05).     <p>&nbsp;</p> <a name="t1"> <img src="/img/revistas/nut/n14/n14a02t1.jpg">     
<p>&nbsp;</p> <br/><u>Pr&#225;tica Desportiva</u>: Verificou-se que 69,9% (n=197) das crian&#231;as inclu&#237;das neste estudo n&#227;o estavam inscritas num clube desportivo. No entanto, este n&#250;mero diminuiu para 65,4% (n=134) ap&#243;s a interven&#231;&#227;o. <body><![CDATA[<p>&nbsp;</p> Ap&#243;s a interven&#231;&#227;o, as crian&#231;as que despendiam menos de duas horas por dia a ver televis&#227;o/utilizar o computador durante a semana, foram as que diminu&#237;ram mais de p de IMC/idade (83,7%), sendo as diferen&#231;as estatisticamente significativas (p<0,05) (<a href="#t3">Tabela 3</a>). <br/><u>Horas de sono di&#225;rias</u>: No que concerne ao n&#250;mero de horas de sono di&#225;rias, a maioria das crian&#231;as (52,8%) dormia entre nove a dez horas por dia durante a semana, sendo que durante o fim-de-semana observou-se que mais de metade das crian&#231;as (65,8%) dormia dez ou mais horas por dia. A maior diminui&#231;&#227;o de p de IMC/idade (83,1%) ocorreu nas crian&#231;as que dormiam dez ou mais horas por dia durante o fim-de-semana, ap&#243;s a interven&#231;&#227;o, apesar de as diferen&#231;as n&#227;o serem estatisticamente significativas (p>0,05) (<a href="#t4">Tabela 4</a>).     <p>&nbsp;</p> <a name="t4"> <img src="/img/revistas/nut/n14/n14a02t4.jpg">     
<p>&nbsp;</p> <br/> <br/><b >Discuss&#227;o</b> <br/>O POZ teve resultados positivos em termos de altera&#231;&#227;o do p de IMC nas crian&#231;as, j&#225; que 80,4% das crian&#231;as diminu&#237;ram o seu p de IMC/idade (p<0,05). Estes resultados mostram que as interven&#231;&#245;es de base comunit&#225;ria e familiar s&#227;o eficazes na abordagem da obesidade infantil, tal como tem sido demonstrado por outros autores (5,9,10,22). A pr&#225;tica de exerc&#237;cio f&#237;sico &#233; essencial no combate do excesso de peso infantil (5, 7, 9). Cerca de 69,9% das crian&#231;as inclu&#237;das neste estudo n&#227;o estavam inscritas num clube desportivo. No entanto, ap&#243;s a interven&#231;&#227;o, este n&#250;mero diminuiu para 65,4%, revelando melhorias comportamentais tal como no estudo de Sacher et al. (23). No que concerne aos dias por semana que as crian&#231;as frequentavam um clube desportivo, verificou-se um aumento do n&#250;mero de crian&#231;as que frequentavam o mesmo dois ou mais dias por semana, o que vai de encontro &#224;s recomenda&#231;&#245;es da OMS (12) e do CDC (13). &#201; ainda de salientar que as crian&#231;as que participavam num clube desportivo dois ou mais dias por semana foram aquelas em que ocorreu uma maior diminui&#231;&#227;o de p de IMC, estando estes resultados em concord&#226;ncia com os de Lazaar et al. (24) e Buchan et al. (25). Verificou-se que durante a semana, 64,2% das crian&#231;as passava menos de duas horas por dia a utilizar a televis&#227;o e/ou o computador o que vai de encontro &#224;s recomenda&#231;&#245;es da OMS e do CDC (12,13). Na 4.&#170; COI, verificou-se que as crian&#231;as que despendiam menos de duas horas por dia durante a semana a ver televis&#227;o ou a utilizar o computador foram as que diminu&#237;ram mais de p de IMC/idade (p<0,05), tal como nos estudos de Davidson et al. (26) e Gentile et al. (27). O n&#250;mero de horas de sono tem sido cada vez mais associado ao excesso de peso infantil (16, 18). Tem-se verificado que quando as crian&#231;as dormem poucas horas durante a noite, o risco de se tornarem adolescentes ou at&#233; mesmo adultos com excesso de peso &#233; maior (28). Verificou-se que durante a semana a maioria das crian&#231;as (52,8 %) dormia entre nove a dez horas por dia, n&#227;o atingindo as recomenda&#231;&#245;es de 10-11h/dia (15), muito embora o n&#250;mero de horas de sono di&#225;rias tenha sido superior durante o fim-de-semana, estando estes resultados de acordo com os obtidos por Wing et al. (29). 83,1% das crian&#231;as que dormiam dez ou mais horas por dia durante o fim-de-semana diminuiram de p de IMC, o que vai de encontro &#224;s altera&#231;&#245;es observadas por Rutters et al. (28). <br/>Este estudo apresenta algumas limita&#231;&#245;es. A maior parte das crian&#231;as que n&#227;o participaram no projecto foram crian&#231;as cujas fam&#237;lias recusaram a participa&#231;&#227;o no mesmo, apesar de ser um projecto totalmente gratuito para as fam&#237;lias. Ainda a este respeito, salientando-se a escassez de respostas ao question&#225;rio dirigido aos pais das crian&#231;as relativamente &#224; pr&#225;tica de actividades desportivas, aos comportamentos sedent&#225;rios e ao n&#250;mero de horas de sono di&#225;rias, que poder&#225; estar associado ao facto do question&#225;rio ser muito extenso, tornando-o, por vezes cansativo e levando as pessoas a evitar respostas que n&#227;o fossem de op&#231;&#227;o de resposta dicot&#243;mica. Ao mesmo tempo o drop-out verificado, ainda que pequeno (7,5%) pode ter influenciado os resultados. A abordagem desta quest&#227;o &#224;s fam&#237;lias carece ainda de uma metodologia melhor definida.</p>     <p><b >Conclus&#245;es</b> <br/>Este estudo sugere que os projectos de base comunit&#225;ria e familiar s&#227;o relevantes na abordagem da obesidade infantil, enfatizando a import&#226;ncia do aumento da pr&#225;tica da actividade desportiva e do n&#250;mero de horas de sono di&#225;rias bem como da redu&#231;&#227;o dos comportamentos sedent&#225;rios na melhoria do estado nutricional das crian&#231;as.</p>     <p>&nbsp;</p>     <!-- ref --><p><b>Refer&#234;ncias Bibliogr&#225;ficas</b><b ></b> <br/>1.
