Um aporte de proteína, nutriente fundamental em todas as fases do processo de cicatrização, ajustado a cada situação é fundamental não existindo contudo consenso sobre o assunto. A arginina e a glutamina, dois aminoácidos envolvidos em várias etapas do processo de cicatrização, tornam-se condicionalmente essenciais em situações de stress. Vários autores descreveram benefícios na suplementação com estes nutrientes, no entanto são necessários mais estudos de forma a avaliar o efeito e a eficácia da suplementação na cicatrização de feridas assim como estabelecer doses terapêuticas eficazes e seguras. A suplementação pode ser vantajosa em indivíduos com deficiências nutricionais, desnutridos ou num estado catabólico severo mas não substitui uma alimentação completa, equilibrada e variada.]]></p></abstract>

A altera&#231;&#227;o no processo de cicatriza&#231;&#227;o das feridas que resulte num atraso ou falha da mesma leva &#224; diminui&#231;&#227;o da qualidade de vida do doente e dos seus cuidadores e a um acr&#233;scimo em termos de morbilidade e mortalidade constituindo uma insustent&#225;vel sobrecarga econ&#243;mica para os servi&#231;os de sa&#250;de (2-4). <br/>&#201; consensual a import&#226;ncia da nutri&#231;&#227;o na preven&#231;&#227;o e no tratamento das feridas (2,5-8). As defici&#234;ncias nutricionais e a desnutri&#231;&#227;o antes e durante o processo de cura podem atrasar o processo de cicatriza&#231;&#227;o e comprometer um tratamento adequado da ferida (2,4-7). <br/>Actualmente sabe-se tamb&#233;m que &#233; necess&#225;rio um aporte adequado de prote&#237;na, pois esta &#233; necess&#225;ria em todas as fases da cicatriza&#231;&#227;o (4,7), no entanto as recomenda&#231;&#245;es diferem entre v&#225;rios autores. Nesta revis&#227;o pretende-se abordar de forma gen&#233;rica a import&#226;ncia da nutri&#231;&#227;o na cicatriza&#231;&#227;o de feridas e em pormenor o papel das prote&#237;nas e de alguns amino&#225;cidos.  <br/> <br/><b >Processo de cicatriza&#231;&#227;o de feridas</b> <br/>Uma ferida pode ser definida como uma solu&#231;&#227;o de continuidade da pele ou dos seus anexos, dos tecidos moles ou das paredes das cavidades org&#226;nicas (9). A cicatriza&#231;&#227;o da mesma &#233; um processo complexo dividido em v&#225;rias fases que se podem sobrepor em diferentes locais da ferida assim como podem ser inibidas ou ser influenciadas negativamente por v&#225;rios factores intr&#237;nsecos e extr&#237;nsecos. O estado nutricional e o aporte energ&#233;tico e de nutrientes s&#227;o dois deles (2,5,8,10-12). Na <a href="#f1">Figura 1</a> est&#227;o representados alguns factores que afectam a cicatriza&#231;&#227;o de feridas (1,10-12).     <p>&nbsp;</p> <a name="f1"> <img src="/img/revistas/nut/n14/n14a07f1.jpg">

<body><![CDATA[<p>&nbsp;</p> <br/><u> </u> <br/><b >Nutri&#231;&#227;o e cicatriza&#231;&#227;o de feridas</b> <br/>Uma ferida causa in&#250;meras mudan&#231;as no organismo, proporcionais &#224; sua severidade, decorrentes de um processo catab&#243;lico que leva ao aumento das necessidades energ&#233;ticas e nutricionais (7,8).  <br/>A repara&#231;&#227;o e a reconstru&#231;&#227;o dos tecidos, que ocorre durante a cicatriza&#231;&#227;o, requerem quantidades adequadas de energia e de nutrientes. Normalmente estes substratos s&#227;o fornecidos pelas reservas do organismo, no entanto indiv&#237;duos desnutridos t&#234;m esta capacidade comprometida influenciando o processo de cicatriza&#231;&#227;o (7,8). Tamb&#233;m em indiv&#237;duos saud&#225;veis com feridas e com uma ingest&#227;o inadequada relativamente &#224;s necessidades energ&#233;ticas e nutricionais aumentadas, e assim em risco de desnutri&#231;&#227;o, podem ter o processo de cicatriza&#231;&#227;o comprometido. Indiv&#237;duos em risco de desenvolver &#250;lceras de press&#227;o parecem ter um risco de desnutri&#231;&#227;o 2 a 3x superior (13).  <br/>A desnutri&#231;&#227;o proteico-energ&#233;tica &#233; a mais comum em indiv&#237;duos com feridas (14). Na <a href="#f2">Figura 2</a> est&#227;o representadas algumas altera&#231;&#245;es que ocorrem na desnutri&#231;&#227;o e influ&#234;ncia na cicatriza&#231;&#227;o de feridas (14).      <p>&nbsp;</p> <a name="f2"> <img src="/img/revistas/nut/n14/n14a07f2.jpg">     
<p>&nbsp;</p> <br/>A melhoria do estado nutricional permite que o organismo cicatrize as feridas e poder&#225; mesmo acelerar o processo de cicatriza&#231;&#227;o (8). <br/><u> </u> <br/><b >Prote&#237;na e cicatriza&#231;&#227;o de feridas</b> <br/>As prote&#237;nas desempenham um papel importante em todo o processo de cicatriza&#231;&#227;o (7, 8, 15). Desde a fase inflamat&#243;ria, passando pela fase proliferativa at&#233; &#224; fase de remodela&#231;&#227;o, as prote&#237;nas s&#227;o utilizadas como substrato mas tamb&#233;m como mediadores inflamat&#243;rios (enzimas proteol&#237;ticas, citocinas, neuropept&#237;deos) de todo o processo (16-20). Um evento chave na gera&#231;&#227;o de v&#225;rios mediadores da inflama&#231;&#227;o &#233; a activa&#231;&#227;o do factor de Hageman (factor XII do sistema de coagula&#231;&#227;o), uma prote&#237;na produzida no f&#237;gado. A sua activa&#231;&#227;o inicia quatro sistemas envolvidos na resposta inflamat&#243;ria: o sistema das cininas, produzindo cininas vasoactivas; o sistema de coagula&#231;&#227;o, induzindo a activa&#231;&#227;o da trombina, fibrinopept&#237;deos e factor X, todos com propriedades inflamat&#243;rias; o sistema fibrinol&#237;tico, produzindo plasmina e inactivando a trombina; e o sistema do complemento, produzindo as anafilatoxinas C3a e C5a (21).  <br/>A deple&#231;&#227;o de prote&#237;na prolonga o tempo da fase inflamat&#243;ria, inibe a prolifera&#231;&#227;o fibrobl&#225;stica e a angiog&#233;nese, diminui a s&#237;ntese e deposi&#231;&#227;o de colag&#233;nio e proteoglicanos, reduz a for&#231;a t&#234;nsil da ferida, condiciona a capacidade fagoc&#237;tica dos leuc&#243;citos e a resposta imune e inibe a remodela&#231;&#227;o da ferida (16, 22, 23). <br/>A queimadura representa provavelmente o maior est&#237;mulo para o catabolismo muscular proteico. As perdas aumentadas de azoto est&#227;o associadas a um maior risco de morbi-mortalidade na fase aguda, e podem atingir valores acima de 40 g/dia em doentes queimados graves alimentados, ou seja, 10 vezes superior &#224; excretada por pessoas saud&#225;veis em jejum proteico (4,1g/dia) (24). A resposta &#224; queimadura &#233; proporcional &#224; sua extens&#227;o e o dano pode manter-se durante anos (25). Traumas severos ou sepsis podem aumentar a perda de prote&#237;na para 150 a 250g (600 a 1000g massa muscular) por dia (8,25). <br/>Tamb&#233;m um aporte energ&#233;tico insuficiente causa deple&#231;&#227;o de prote&#237;na pois as reservas proteicas funcionam como fonte energ&#233;tica levando &#224; perda de massa magra (15,16). &#192; medida que a % de massa magra diminui mais prote&#237;na &#233; utilizada para repor as perdas dessa mesma massa, comprometendo assim a cicatriza&#231;&#227;o das feridas (16). <br/>Na <a href="#t1">Tabela 1</a> est&#227;o descritas as complica&#231;&#245;es resultantes da perda de massa corporal magra (16).     <p>&nbsp;</p> <a name="t1"> <img src="/img/revistas/nut/n14/n14a07t1.jpg">     
<p>&nbsp;</p>  <br/>Pelo aumento das necessidades e pela perda no exsudado torna-se crucial a determina&#231;&#227;o de um valor adequado para a ingest&#227;o proteica, de acordo com as diversas situa&#231;&#245;es, n&#227;o existindo contudo consenso sobre a mat&#233;ria. <br/>Na <a href="#t2">Tabela 2</a> est&#227;o descritas as recomenda&#231;&#245;es de ingest&#227;o proteica de acordo com o estado dos indiv&#237;duos (1,4,16,24-28,38).     <p>&nbsp;</p> <a name="t2"> <img src="/img/revistas/nut/n14/n14a07t2.jpg">     
<p>&nbsp;</p>     <br/>De acordo com Pompeo (2007) (29), a sua experi&#234;ncia no     tratamento de feridas severas demonstrou que estas recomenda&#231;&#245;es s&#227;o     insuficientes para normalizar as reservas proteicas. Segundo o autor, o <i     ]]></body>
<body><![CDATA[>Pressure Ulcer Scale for Healing</i> (PUSH)     parece ser a melhor ferramenta para calcular as necessidades proteicas pois     este avalia factores adicionais como a quantidade de exsudado e a presen&#231;a de     tecido n&#227;o vi&#225;vel (4,29). O valor dever&#225;, no entanto, ser ajustado de acordo     com a evolu&#231;&#227;o ponderal, dieta e n&#237;vel de pr&#233;-albumina. De acordo com a     pontua&#231;&#227;o do PUSH, no momento da admiss&#227;o, o autor recomenda os seguintes     valores (29):      <br/>&#8226; Pontua&#231;&#227;o do PUSH de 0 a 15 = 1,4 to 1,6g     <br/>prote&#237;nas/kg peso corporal/dia     <br/>&#8226; Pontua&#231;&#227;o do PUSH de 16 a 30 = 1,6 to 2,0     ]]></body>
<body><![CDATA[<br/>prote&#237;nas/kg peso corporal/dia     <br/>&#8226; Pontua&#231;&#227;o do PUSH superior a 30 = 2,0 to 2,4     <br/>prote&#237;nas/kg peso corporal/dia     <br/>Diversos estudos sugerem que a sobrecarga em prote&#237;nas     ou em mistura de amino&#225;cidos pode causar les&#227;o glomerular e acelerar a     progress&#227;o de doen&#231;as renais cr&#243;nicas preexistentes (23,24). Assim &#233;     fundamental monitorizar a fun&#231;&#227;o renal para assegurar que o aumento na ingest&#227;o     de prote&#237;na est&#225; adequado ao indiv&#237;duo (4,23).     <br/>Existe a evid&#234;ncia que alguns amino&#225;cidos s&#227;o     essenciais ao processo de cicatriza&#231;&#227;o de feridas, entre eles a ciste&#237;na, a     ]]></body>

Nesta revis&#227;o ser&#227;o descritas a     arginina e a glutamina, visto serem estes amino&#225;cidos que apresentam mais     estudos na suplementa&#231;&#227;o em doentes com feridas.     <br/><u> </u>     <br/><b >Arginina</b>     <br/>Em situa&#231;&#245;es normais, o organismo consegue sintetizar     arginina em quantidade adequada para manter a integridade do m&#250;sculo e tecidos     mas sob stress ou na presen&#231;a de uma ferida, as reservas deste amino&#225;cido     decrescem rapidamente. Nestes momentos, em que a s&#237;ntese &#233; insuficiente, a     arginina torna-se num amino&#225;cido condicionalmente essencial para o processo de     ]]></body>
<body><![CDATA[cicatriza&#231;&#227;o e na manuten&#231;&#227;o de um balan&#231;o azotado positivo (16, 30-32).     <br/>A arginina est&#225; envolvida em v&#225;rias etapas do processo     de cicatriza&#231;&#227;o nomeadamente pelo papel na s&#237;ntese proteica e manuten&#231;&#227;o de um     balan&#231;o azotado positivo, como precursora da prolina na s&#237;ntese de colag&#233;nio,     pela capacidade em induzir a produ&#231;&#227;o de certas hormonas (como a hormona de     crescimento humana) que medeiam os mecanismos de cicatriza&#231;&#227;o e pela     estimula&#231;&#227;o da resposta imunit&#225;ria (16, 30, 31, 33-37). &#201;, ainda, importante     referir que a arginina pode ser metabolizada em mon&#243;xido de azoto (NO) e     citrulina, pela via da sintetase de NO. O NO possui um papel importante na     vasodilata&#231;&#227;o, na resposta inflamat&#243;ria, na angiog&#233;nese, na prolifera&#231;&#227;o     ]]></body>
<body><![CDATA[celular e na deposi&#231;&#227;o da matriz (30, 38).     <br/>Segundo alguns autores a suplementa&#231;&#227;o com arginina     diminui a perda de massa muscular, melhora a s&#237;ntese de colag&#233;nio e &#233;     importante para a deposi&#231;&#227;o do mesmo no processo de cicatriza&#231;&#227;o, melhora a     resposta imunit&#225;ria e estimula a secre&#231;&#227;o de hormonas anab&#243;licas, como a     hormona de crescimento humano, tamb&#233;m envolvidas na cicatriza&#231;&#227;o (30-32). Zhang     et al. (2008) (38), a partir de um estudo feito em animais, descreveram que,     apesar do NO desempenhar importantes fun&#231;&#245;es no processo de cicatriza&#231;&#227;o, o     est&#237;mulo no anabolismo proteico, provocado pela suplementa&#231;&#227;o com arginina, foi     independente da produ&#231;&#227;o de NO.      ]]></body>
<body><![CDATA[<br/>A suplementa&#231;&#227;o com arginina, em contexto cl&#237;nico, &#233;     geralmente segura, no entanto dever&#225; ser determinada uma dose terap&#234;utica     m&#225;xima segura. Benati et al. (2001) (39) verificaram que a suplementa&#231;&#227;o com 9g     de arginina promove a cicatriza&#231;&#227;o no tratamento de &#250;lceras de press&#227;o. Witte     et al. (2003) (31) descreveram alguns benef&#237;cios na suplementa&#231;&#227;o com arginina,     independentemente da via de administra&#231;&#227;o. Kirk et al. (1993) (40) verificaram     uma melhoria na cicatriza&#231;&#227;o de feridas (experimentais) e na resposta imune em     idosos suplementados com 17g de arginina durante 2 semanas, n&#227;o tendo sido     detectados efeitos adversos. Ahuja et al. (2007) (30) referem que a maioria dos     indiv&#237;duos consegue tolerar doses elevadas de arginina (30 a 60g/dia), embora     ]]></body>
<body><![CDATA[aqueles com insufici&#234;ncia hep&#225;tica ou renal devem ser cuidadosamente     monitorizados.     <br/>Apesar de v&#225;rios estudos mostrarem os benef&#237;cios da     suplementa&#231;&#227;o de arginina na cicatriza&#231;&#227;o de feridas s&#227;o necess&#225;rios mais     estudos para determinar a dose m&#225;xima de suplementa&#231;&#227;o com arginina em humanos     assim como a efic&#225;cia da suplementa&#231;&#227;o apenas com arginina ou combinada com     outros nutrientes no tratamento de feridas (4).     <br/><u> </u>     <br/><b >Glutamina</b>     <br/>A glutamina &#233; o amino&#225;cido mais abundante no     ]]></body>
<body><![CDATA[organismo. Em situa&#231;&#245;es de stress metab&#243;lico a sua concentra&#231;&#227;o plasm&#225;tica     diminui rapidamente (2, 41, 42).     <br/>A glutamina &#233; usada na neoglicog&#233;nese, na produ&#231;&#227;o e     prolifera&#231;&#227;o linfocit&#225;ria (substrato energ&#233;tico para os linf&#243;citos) e tamb&#233;m na     estimula&#231;&#227;o da resposta imunit&#225;ria na cicatriza&#231;&#227;o. Possui, ainda propriedades     anab&#243;licas e anticatab&#243;licas, sendo precursor na s&#237;ntese de purinas, pirimidinas     e de fosfol&#237;pidos, e promove a integridade do intestino (2,16,41,42).     <br/>A maioria dos benef&#237;cios verificados pela     suplementa&#231;&#227;o com glutamina parecem estar relacionados com a melhoria da     integridade intestinal, normaliza&#231;&#227;o dos n&#237;veis s&#233;ricos, melhoria na s&#237;ntese     ]]></body>
<body><![CDATA[proteica e diminui&#231;&#227;o no n&#250;mero de dias de internamento hospitalar (2, 43).     <br/>Tal como para outros nutrientes, n&#227;o existe um valor     de refer&#234;ncia para a suplementa&#231;&#227;o com glutamina. A maioria dos estudos mostram     benef&#237;cios usando valores iguais ou inferiores a 0,6g de glutamina/kg peso     corporal/dia (16, 42), no entanto de acordo com o <i >National Pressure Ulcer Advisory Panel White Paper</i> a dose m&#225;xima     segura ser&#225; 0,57g/kg/dia (4).     <br/>De acordo com Novak et al. <body><![CDATA[Schoemann et al. (2007) (42) referem que a suplementa&#231;&#227;o com 0,5 g de     glutamina/kg peso corporal/dia no tratamento de feridas parece adequada. A     suplementa&#231;&#227;o deve ser usada com precau&#231;&#227;o em idosos e em doentes com     hiperamonemia e insufici&#234;ncia hep&#225;tica ou renal (2, 16, 42).     <br/>Mais estudos dever&#227;o ser feitos para concluir a     efic&#225;cia da suplementa&#231;&#227;o de glutamina na cicatriza&#231;&#227;o de feridas assim como     estabelecer doses terap&#234;uticas adequadas.     <br/>     <p><b >An&#225;lise cr&#237;tica</b> <br/>O papel da nutri&#231;&#227;o na preven&#231;&#227;o e no tratamento de feridas est&#225; reconhecido. Uma dieta completa, equilibrada e variada &#233; fundamental para garantir um correcto aporte energ&#233;tico e proteico, no entanto em indiv&#237;duos com defici&#234;ncias nutricionais, desnutridos ou num estado catab&#243;lico severo pode n&#227;o ser suficiente. Nestes casos poder&#225; ser necess&#225;ria uma suplementa&#231;&#227;o para promover a cicatriza&#231;&#227;o das feridas.  <br/>Em indiv&#237;duos bem nutridos, a suplementa&#231;&#227;o com nutrientes como prote&#237;nas, arginina, vitamina A, C, E, zinco, cobre e sel&#233;nio, tamb&#233;m deve ser considerada pois as necessidades est&#227;o aumentadas e estes s&#227;o necess&#225;rios como substrato da ferida e para a repara&#231;&#227;o e regenera&#231;&#227;o tecidular. Anholt et al. (2010) (13) verificaram que em indiv&#237;duos adultos bem nutridos com &#250;lceras de press&#227;o (est&#225;dio III a IV) e sem outras complica&#231;&#245;es severas, suplementados com prote&#237;na, arginina, vitamina A, vitamina C, vitamina E, zinco, sel&#233;nio, cobre e &#225;cido f&#243;lico, houve diminui&#231;&#227;o do tempo de cicatriza&#231;&#227;o, da severidade (pontua&#231;&#227;o do PUSH) e do exsudado por semana, melhoria na qualidade de vida e diminui&#231;&#227;o dos custos com o tratamento. <br/>Estudos randomizados e maiores dever&#227;o ser realizados de forma a avaliar o efeito, a efic&#225;cia e o risco da suplementa&#231;&#227;o de prote&#237;na e de amino&#225;cidos como a glutamina e a arginina na cicatriza&#231;&#227;o de feridas assim como estabelecer doses terap&#234;uticas eficazes e seguras.</p>     <p><b >Conclus&#245;es</b> <br/>O papel da nutri&#231;&#227;o, nomeadamente da prote&#237;na, na cicatriza&#231;&#227;o das feridas &#233; evidente. A combina&#231;&#227;o do conhecimento de um tratamento correcto da ferida em combina&#231;&#227;o com um bom aporte nutricional pode resultar na diminui&#231;&#227;o da morbilidade e mortalidade, na melhoria na qualidade de vida e na diminui&#231;&#227;o do tempo de internamento e do n&#250;mero de admiss&#245;es hospitalares.  <br/>Assim, &#233; fundamental a presen&#231;a de um nutricionista, na equipa multidisciplinar que fa&#231;a uma correcta avalia&#231;&#227;o e diagn&#243;stico nutricional para que a interven&#231;&#227;o nutricional, de acordo com as recomenda&#231;&#245;es dispon&#237;veis, permita satisfazer as necessidades aumentadas de energia, prote&#237;na e micronutrientes, previna a desnutri&#231;&#227;o proteico-energ&#233;tica e promova a cicatriza&#231;&#227;o das feridas. <br/>O suporte nutricional deve ser personalizado de acordo com as necessidades energ&#233;ticas e nutricionais, estado cl&#237;nico do doente, e suas prefer&#234;ncias e gostos. O doente e a fam&#237;lia ou os cuidadores devem ser envolvidos no processo. Quando necess&#225;ria, a suplementa&#231;&#227;o n&#227;o substitui uma alimenta&#231;&#227;o completa, equilibrada e variada.</p>     ]]></body>
