We present the case of a 70-year-old male with end-stage renal disease requiring central venous access for hemodialysis. Despite ultrasound-guided insertion, the CVC was inadvertently advanced into the left superior pulmonary vein, leading to the presence of arterialized blood on aspiration. Chest radiography initially suggested an atypical catheter trajectory, and computed tomography confirmed the anomalous connection of the left superior pulmonary vein to the left brachiocephalic vein, consistent with a partial anomalous pulmonary venous drainage. The discussion highlights the imaging findings, differential diagnosis, and clinical implications of mispositioned CVCs in patients with vascular anomalies.]]></p></abstract>
<abstract abstract-type="short" xml:lang="pt"><p><![CDATA[Resumo Os cateteres venosos centrais (CVCs) são amplamente utilizados na prática clínica, mas o seu posicionamento anómalo pode resultar em complicações graves. Apresentamos o caso de um doente do sexo masculino de 70 anos, com doença renal terminal que necessitava de um acesso venoso central para hemodiálise. Apesar da inserção guiada por ecografia, o CVC foi inadvertidamente posicionado na veia pulmonar superior esquerda, resultando na aspiração de sangue arterializado. A radiografia torácica sugeriu um trajeto atípico do cateter, posteriormente confirmado por tomografia computorizada, que evidenciou uma conexão anómala da veia pulmonar superior esquerda à veia braquiocefálica esquerda, compatível com drenagem venosa pulmonar anómala parcial. A discussão aborda os achados imagiológicos, o diagnóstico diferencial e implicações clínicas dos CVCs mal posicionados em doentes com anomalias vasculares.]]></p></abstract>
