Methods: A retrospective cross-sectional study was conducted using a random sample of 360 computed tomography examinations requested in 2023 at the basic emergency department of Lamego Hospital. Data collected included demographic and clinical information, exam characteristics, and the medical specialty of the requesting physician. Imaging findings were categorised accordingly. Appropriateness and relevance of findings were independently evaluated by two certified radiologists. The association between appropriateness and findings was analysed using descriptive statistics and logistic regression. Results: A total of 360 examinations from 342 patients were analysed, with a median age of 73 years; 51.5% were female. Most examinations were requested for patients triaged as Urgent (70.3%), predominantly by physicians from General Surgery, General Medicine, and Internal Medicine (96.4%). Overall, 26.9% of examinations were deemed inappropriate, with higher rates observed in younger, male patients and those classified as Green triage level (45%). Appropriately requested examinations were significantly more likely to detect findings relevant to the emergency admission. Conclusions: A substantial proportion of exams in this emergency department were inappropriately requested, consistent with international evidence. Appropriately requested examinations were up to six times more likely to yield findings relevant to the patient&#8216;s emergency condition, underscoring the importance of sound clinical reasoning, adherence to guidelines, and the use of decision-support tools such as the ESR iGuide.]]></p></abstract>
<abstract abstract-type="short" xml:lang="pt"><p><![CDATA[Resumo Objetivo: Avaliar a adequação dos pedidos de tomografia computadorizada num serviço de urgência básica e correlacionar com os respetivos achados imagiológicos. Métodos: Estudo transversal retrospetivo com uma amostra aleatória de 360 exames de tomografia computorizada solicitados em 2023 no serviço de urgência básica do Hospital de Lamego. Foram recolhidos dados demográficos, clínicos, características do exame e especialidade do médico prescritor. Os achados imagiológicos foram categorizados. A adequação e relevância dos achados foram avaliadas por dois radiologistas certificados. A associação entre adequação e achados foi analisada através de estatísticas descritivas e regressão logística. Resultados: Foram analisados 360 exames de tomografia computorizada de 342 doentes, com idade mediana de 73 anos e 51,5% do sexo feminino. A maioria dos exames foi solicitada para doentes triados como Urgentes (70,3%), essencialmente por médicos de Cirurgia Geral, Clínica Geral e Medicina Interna (96,4%). No total, 26,9% dos exames foram considerados inadequados, com taxas mais elevadas em doentes mais jovens, do sexo masculino e no nível de triagem Verde (45%). Os exames adequadamente solicitados apresentaram maior probabilidade de detetar achados relevantes relacionados com o episódio de urgência. Conclusões: Houve uma proporção substancial de exames inadequadamente solicitados no serviço de urgência, em linha com dados internacionais. Exames adequadamente solicitados apresentaram probabilidade até seis vezes superior de detetar achados relevantes para o episódio de urgência do doente, sublinhando a importância do raciocínio clínico, adesão a guidelines e utilização de ferramentas de suporte à decisão, como o ESR iGuide.]]></p></abstract>
