Em Portugal, esta doença é de Declaração Obrigatória desde 1950. A bactéria tem um período de incubação de sete a dez dias, podendo chegar a vinte e um dias. A transmissão ocorre por via aérea, através da inalação de gotículas respiratórias, sendo possível tanto por sintomáticos quanto assintomáticos. Nem a infeção prévia, nem a vacinação fornecem imunidade permanente, embora esta última tenha reduzido drasticamente a incidência da doença.  Objetivos: Desenvolvimento de um protocolo de atuação em caso de exposição laboral à Bordetella pertussis em meio hospitalar.  Metodologia: Análise dos aspetos clínicos e preventivos da tosse convulsa, fundamentada nas diretrizes da Direção-Geral da Saúde, do Centro de Controlo e Prevenção de Doenças e numa revisão da literatura utilizando as bases de dados MEDLINE e PubMed.  Resultados e discussão: Esta patologia é caracterizada por três fases, catarral, paroxística e de convalescença; em adultos, os sintomas são menos intensos e frequentemente limitados a tosse persistente. O exame cultural, apesar de ser o gold-standard para o diagnóstico, tem sensibilidade reduzida em casos de uso recente de antibióticos, vacinação prévia ou doença prolongada, sendo a Proten Chain Reaction o método mais utilizado. Embora a tosse convulsa em adultos seja autolimitada, a antibioterapia precoce, idealmente na fase catarral, reduz a duração e gravidade da tosse e a disseminação da doença. O método de prevenção mais eficaz é a vacinação, sendo que a profilaxia pós-exposição deve ser administrada a todos os profissionais que tiveram contato próximo com pacientes sintomáticos.  Conclusões: Trabalhadores com suspeita de tosse convulsa devem ser avaliados pelo Serviço de Saúde Ocupacional para confirmação do diagnóstico. Se confirmado, o médico do trabalho deve prescrever a antibioterapia e declarar a inaptidão temporária do trabalhador. A prevenção da transmissão de Bordetella pertussis em ambiente hospitalar envolve a vacinação, a administração adequada de profilaxia pós-exposição e o afastamento dos trabalhadores potencialmente infetados. O conhecimento adequado da clínica, abordagem e tratamento, assim como medidas preventivas é, portanto, essencial, sendo também de extrema importância a existência de planos de ação bem definidos.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[ABSTRACT  Introduction: Whooping cough is a respiratory infection caused by the bacteria Bordetella pertussis. In Portugal, this disease has been a Mandatory Declaration since 1950. The bacteria has an incubation period of seven to ten days, reaching twenty-one days. Transmission occurs by air, through inhalation of respiratory droplets, and is possible by both symptomatic and asymptomatic patients. Neither prior infection nor vaccination provides permanent immunity, although the latter has drastically reduced the incidence of the disease.  Objetives: Development of a protocol for action in case of occupational exposure to Bordetella pertussis in a hospital environment.  Methodology: Analysis of the clinical and preventive aspects of whooping cough, based on guidelines from the Directorate-General for Health, the Center for Disease Control and Prevention and a literature review using the MEDLINE and PubMed databases.  Results and Discussion: This pathology is characterized by three phases, catarrhal, paroxysmal and convalescent; in adults, symptoms are less intense and often limited to a persistent cough. The cultural exam, despite being the gold-standard for diagnosis, has reduced sensitivity in cases of recent use of antibiotics, previous vaccination or prolonged illness, with the Proten Chain Reaction being the most used method. Although whooping cough in adults is self-limiting, early antibiotic therapy, ideally in the catarrhal phase, reduces the duration and severity of the cough and the spread of the disease. The most effective prevention method is vaccination, and post-exposure prophylaxis should be administered to all professionals who have had close contact with symptomatic patients.  Conclusion: Workers suspected of having whooping cough must be evaluated by the Occupational Health Service to confirm the diagnosis. If confirmed, the occupational physician must prescribe antibiotic therapy and declare the worker temporarily unfit. Preventing the transmission of Bordetella pertussis in a hospital environment involves vaccination, adequate administration of post-exposure prophylaxis and the removal of potentially infected workers. Adequate knowledge of the clinic, approach and treatment, as well as preventive measures is, therefore, essential, and the existence of well-defined action plans is also extremely important.]]></p></abstract>
