To develop clinical practice recommendations and standardize procedures on oral immunotherapy in food allergy in Portugal, the “Food Allergy” Interest Group of the Portuguese Society of Allergology and Clinical Immunology (SPAIC) prepared the “Guidelines on Oral Immunotherapy in Food Allergy”. On this Educational Page we present the fourth chapter of the guidelines, dedicated to oral immunotherapy with cow´s milk, to develop a clinical guide based on the available scientific evidence and the opinion consensus established by a group of experts in this specific intervention. The authors carry out na updated review on the topic, present protocols, and elaborate practical recommendations, covering indications, contraindications, risk factors, cofactors, requirements, and guidance for the long-term follow-up of patients undergoing oral immunotherapy with cow´s milk.
