Synovitis and inflammatory arthritis caused by plant thorn penetrating injuries is an uncommon clinical entity. Its indolent nature leads in many cases to misdiagnosis. The authors report an unusual case of a teenager with inflammatory synovitis of the knee caused by palm thorn injury. Although in a considerable number of cases this synovitis or arthritis is considered aseptic, the most frequently isolated organism is Pantoae agglomerans. In these cases it is essential to proceed with foreign body removal, abundant articular washing, by arthrotomy or arthroscopy, and directed antibiotic therapy. Failure in diagnosis or persistence of intra-articular plant fragments is in most cases disastrous, leading to irreparable damage and significantly compromise the patient functional recovery.
