Achilles chronic rupture is the most frequent lesion in the lower limb, has a bigger predominance in males. Indirect ruptures are the most frequent cause, 73% are intratendinous lesions, 24% in the muscular junction and 4% are insertional ruptures. The chronic rupture is found in 25% of series, chirurgical reparation is a big challenge, to repair the gap there are many different techniques. It does still have some controversies because there are no studies available that show the superiority of one technique above other in terms of durability and strength of the tendon after the reparation. We present the clinical case of a male Caucasian patient with 40 years old, was referred to our Hospital with Achilles chronic rupture. Was operated in 2013 February, we performed a Bosworth’s technique. Postoperatively we made a cast for 12 weeks, resumed work at 6 months postoperatively, and was able to do full weight bearing without restrictions for activities of daily living. Currently has 16 months “follow-up” resumed sport without functional impairment or pain complaints, his weight lost 45 kg, has no scar hypertrophy or calf atrophy, the AOFAS SCORE is 100.
